Our Integrated Team Care (ITC) program is designed to provide the extra support and assistance you need to understand and manage your health, ensuring you can access the care you deserve.
what is a chronic health condition?
A chronic health condition is an illness or disease that has been or is likely to be, present for at least six months. Managing a chronic health condition often requires extra help and support, which is where our ITC program comes in.

Our dedicated ITC team is here to walk alongside you on your health journey, providing personalised support to help you:
- Attend specialist and allied health appointments
- Understand and manage your health condition
- Connect with community services, groups, and other health professionals
- Access equipment and aids to improve your condition
- Arrange transport and accommodation for specialist or allied health appointments

- Providing better treatment and management of chronic conditions
- Improving access to culturally appropriate mainstream primary health care services
- Enabling collaboration and support between the mainstream and Aboriginal and Torres Strait Islander health sector

- Aboriginal and Torres Strait Islander health project officers
- Aboriginal and Torres Strait Islander outreach workers
- Care coordinators
Together, we work to ensure you can access the right care when needed, linking health care across various services and setting clear care pathways.
We also collaborate with mainstream health services to provide culturally appropriate care that respects and understands your unique needs.
